FACE Toolset

FACE enables a wide range of assessment and care planning tools to be developed from a single universe of items. Sets of tools have been developed for a range of care groups, all deriving from the common universe but all tailored to the needs of that care group. Examples of FACE Toolsets include:

• The Core Assessment & Outcomes package for Mental health Services
• The Core Assessment & Outcomes package for Single Assessment/Olders People
• The Core Assessment & Outcomes package for Learning Disabilities
• The Core Assessment & Outcomes package for Young People
• The Core Assessment & Outcomes package for Substance Misuse
• Podiatry Assessment

New packages for forensic services, and care recording are in preparation and are due for release this year.

Tool sets may be supplied as either electronic templates or machine-readable code sets.

The accurate & consistent approach taken by FACE Tools leads to high levels of consistency and therefore good quality data recording. This in turn provides reliable and scientifically valid statistical information on individuals and populations.
