Dr/G.P's Consent Form

 

If you have had surgery or major injury in the last year or any current allergies, illnessess or conditions that requires orthodox medication, you will have to get a Dr/G.P's consent form signed.

Having a signed consent form will save you time if you require one before being treated.

Therapist can refuse to treat you without one if required.
